Since 80% of my work is medical, the best medical reference, specifically for phrasing, is GREGG MEDICAL SHORTHAND MANUAL AND DICTIONARY by Effie Smither. Great outlines and disjoined prefixes and suffixes for anyone doing anything medically oriented.
